Information from the Streamlined Assessment Pilot...

This project targeted older people in Harehills and Gipton, looking at their benefits and if they would like any further support. The organisations which people were referred on to would welcome referrals from other people too. The following documents were helpful for reference by the staff and you may wish to use them or adapt them for your own use.

The referral pathways document - this gives details of where to refer people onto based on information you may gather during a discussion around finances.

The assessment record form used was a handy checklist used internally. The reminder of assessment for the customer was left after each visit. This is a very easy way of reminding people, and letting their carers know what has been done, and could be put in a persons Personal Information File if they had one.

If this is not a service you provide referrals from organisations to the Benefits Service for a full benefits check are always welcome. A referral form is supplied for this service.

A consultation was carried out by the project in the Harehills and Gipton area. This report is available for your information.

The Positive Images Collection is a wonderful collection of photographs of older people in Leeds and is held at the Public Health Resource Centre. The photographs are free to use and you can pop in to the Resource Centre at Armley Park Court on Stanningley Road to choose the images you would like to use.

The collection constantly needs developing and updating, so if your organisation is having an event that could boost positive images of older people in Leeds, contact Heather Dickinson on 0113 2953087 or at heather.dickinson@leedsdesignforhealth.co.uk to arrange for a photographer to attend. Individuals photographed will be asked to sign a permission slip which permits use of the photo for 3 years.
